Default Losing Fish

I have been having a big time problem losing fish. I have been fishing for quite a few years now, and have never had this problem. Smaller fish its not so bad, but when I hook into a lunker I have been losing them when they jump out of the water. I have been setting hook right and all, but I just dont get it. Depending on the size of the worm I use either a 3/0, 4/0, or 5/0 texposure hook. Any ideas what the problem is. Help is greatly appreciated.
